ABSTRACT

OBJECTIVE To predict the development trends of licensed pharmacist staffing in retail pharmacies within the western China and provide reference for the formulation of policies related to licensed pharmacists. METHODS Based on the data of retail pharmacies and licensed pharmacists in the western China from 2016 to 2022, a grey model was constructed to analyze and predict the number development trends of retail pharmacies and licensed pharmacists in the western China from 2023 to 2026. RESULTS Currently, the 1∶1 staffing requirement for licensed pharmacists and retail pharmacies had been met in Shaanxi, Guangxi and Gansu. Based on current trends, Inner Mongolia, Chongqing, Yunnan, and Qinghai were expected to meet the 1∶1 staffing requirement for licensed pharmacists and retail pharmacies between 2023 and 2026. Sichuan and Xinjiang were also expected to meet this requirement in the future. However, there was still a significant gap in Guizhou, Xizang, and Ningxia towards achieving the above goals. CONCLUSIONS There is still a discrepancy between the deployment of licensed pharmacists and the national requirements in certain western provinces. Local authorities should formulate relevant policies according to local circumstances. Regions that have already met or will soon achieve the staffing requirement for licensed pharmacists should continue to enhance the quantity and quality of their licensed pharmacist workforce. In areas where meet this criterion in the short term is not feasible, it is necessary to strengthen the development of the licensed pharmacist workforce, and control the number of new retail pharmacies.

ABSTRACT

OBJECTIVE To analyze the influential factors for potentially inappropriate medication (PIM) in elderly cancer patients. METHODS The data of elderly cancer patients hospitalized in a hospital from January to December 2021 were collected. According to the Beers standard of the American Geriatrics Society in 2019 (hereinafter referred to as the “2019 version of Beers standard”) and Criteria for Potentially Inappropriate Drug Use in Chinese Elderly (2017 version) (hereinafter referred to as the “Chinese PIM standard”), the PIM status of elderly cancer patients was retrospectively analyzed. Multivariate Logistic regression analysis was used to identify influential factors for PIM. RESULTS A total of 293 patients were included in the study. According to the 2019 version of Beers standard, 211 patients (72.01%) had PIM, of which 204 (69.62%) had PIM related to drugs, 6 (2.05%) had PIM related to diseases or symptoms, 46 (15.70%) had PIM that should be used with caution, 32 (10.92%) had PIM with drug-drug interaction that should be avoided, and 11 (3.75%) had PIM based on renal function; the top 5 drugs in the list of incidence were proton pump inhibitors, metoclopramide, the first-generation antihistamines as promethazine, analgesics as ibuprofen and megestrol. According to the Chinese PIM standard, there were 132 patients (45.05%) with PIM, of which 119 (40.61%) had PIM related to drugs, involving 25 drugs (included 7 high-risk drugs and 18 low-risk drugs), and 24 (8.19%) with PIM in disease status; top 4 drugs in the list of incidence were promethazine, megestrol, ibuprofen and cimetidine. Multivariate Logistic regression analysis showed that compared with patients with hospital stay≤10 days, patients with hospital 20054) stay between 11 and 30 days had a higher risk of PIM [odds ratio (OR)=8.836 8, 95% confidence interval (CI) (3.217 8, 31.940 9), P=0.000 1]; compared with the patients with the 65895198。E-mail:fjman@cmpt.ac.cn number of clinical disease diagnosed≤5, patients with the number of clinical disease diagnosed≥11 had a higher risk of PIM [OR=10.930 1, 95%CI (3.000 9, 70.922 9), P=0.001 8]; compared with surgical treatment, patients receiving antineoplastic drugs had a higher risk of PIM [OR=2.209 5, 95%CI (1.180 2, 4.176 9), P=0.013 6]. CONCLUSIONS Elderly cancer patients have multiple diseases, complicated medication, and a high incidence of PIM. The length of hospital stay (11-30 d), the number of clinical disease diagnosed (≥11) and anti-tumor drugs are main influential factors for PIM in patients.

ABSTRACT

Objective:To explore the effect of abdominal breathing exercises at different periods on the gastrointestinal symptoms, quality of life and proton pump inhibitor dependency in patients with gastroesophageal reflux disease (GERD). To provide reference for patients to choose the best time for abdominal breathing exercises.Methods:This was a prospective study. From March 2020 to December 2021, totally 108 GERD patients were collected in digestive outpatient clinic of Yibin Hospital of Chinese Medicine Hospital by convenient sampling method, they were randomly divided into pre meal group, 1 h postprandial group and 2 h postprandial group with 36 cases in each group. All patients in the three groups were given abdominal breathing training for 8 weeks on the basis of conventional acid suppression drug treatment and nursing. The training time of pre meal group, 1 h postprandial group and 2 h postprandial group was at 30 min before meal, 1 h after meal, 2 h after meal, respectively. Before and after 8 weeks of intervention, the difference of gastrointestinal symptoms and quality of life were assessed by Reflux Disease Ouestionnaire (RDQ) and the MOS 36 Item Short Form Health Survey (SF-36). Patients followed up for 12 weeks after drug withdrawal, the medication of proton pump inhibitor (PPI) between three groups were compared.Results:There was no significant difference in RDQ score and SF-36 score among the three groups before intervention ( P>0.05). After intervention, the symptom scores were (7.89 ± 1.86) in the 1 h postprandial group, lower than in the pre meal group (10.38 ± 1.81) and in the 2 h postprandial group (9.64 ± 1.65), the difference was statistically significant ( t = 5.83, 4.06, both P<0.01). The scores of each demensions in SF-36 were higher in the 1 h postprandial group compared to the pre meal group and the 2 h postprandial group, the differences were statistically significant ( t values were 2.04-3.70, all P<0.05). After followed up for 12 weeks, the PPI discontinuation rate was 71.43% in the 1 h postprandial group, higher than in the pre meal group 44.12% and in the 2 h postprandial group 45.45%, the differences were statistically significant ( χ2 = 5.28, 4.73, both P<0.05). Conclusions:Abdominal breathing exercises at 1 hour after meal can effectively alleviate gastrointestinal symptoms, promote quality of life and decrease the proton pump inhibitor dependency of GERD patients.

ABSTRACT

ObjectiveTo explore the mechanism of plumbagin as a novel ferroptosis inducer in bladder cancer inhibition. MethodBladder cancer T24 cells were used in this study. The effect of different concentrations of plumbagin (0.1, 1, 2, 3, 6, 12, 24, 48 μmol·L-1) on the viability of T24 cells was detected by cell counting kit-8 (CCK-8). The effect of different concentrations of plumbagin (1.5, 3, 6 μmol·L-1) on the apoptosis of T24 cells was detected by annexin V-fluorescein isothiocyanate (Annexin V FITC)/PI apoptosis kit. Different inhibitors (ferroptosis inhibitor Fer-1, apoptosis inhibitor VAD, and necroptosis inhibitor Nec-1) were used in combination with plumbagin (6 μmol·L-1). Reactive oxygen species (ROS) fluorescent probe (DCFH-DA), malonaldehyde (MDA), and glutathione (GSH) kits were used to detect the effects of different concentrations of plumbagin (1.5, 3, 6 μmol·L-1) on the level of ROS and the content of MDA and GSH in T24 cells, respectively. The effect of different concentrations of plumbagin (1.5, 3, 6 μmol·L-1) on peroxide levels in T24 cells was detected by C11-BODIPY fluorescent probe. Western blot was used to detect the effect of different concentrations of plumbagin (1.5, 3, 6 μmol·L-1) on the protein expression of solute carrier family 7 member 11 (SLC7A11), glutathione peroxidase 4 (GPX4), nuclear factor E2-related factor-2 (Nrf-2), and Kelch-like ECH-associated protein 1 (Keap1). ResultCompared with the blank group, plumbagin could inhibit the activity of T24 cells (P<0.05) with IC50 of 3.52 μmol·L-1. At the concentrations of 1.5, 3, 6 μmol·L-1, plumbagin significantly promoted the apoptosis of T24 cells (P<0.05) as compared with the blank group. Compared with the plumbagin group at 6 μmol·L-1, the ferroptosis inhibitor and apoptosis inhibitor groups could reverse the inhibitory effect of 6 μmol·L-1 plumbagin on the proliferation of T24 cells (P<0.05). Compared with the blank group, the plumbagin groups at 1.5, 3, 6 μmol·L-1 showed increased content of ROS, MDA, and lipid peroxides in T24 cells, decreased GSH level, and reduced SLC7A11, GPX4, and Nrf-2/Keap1 (P<0.05). Conclusionplumbagin can induce ferroptosis, and its mechanism is related to the Nrf-2/Keap1 signaling pathway.

ABSTRACT

This paper summarized the implementation of the pharmacist examination outline of the seventh edition of the national professional qualification examination for licensed pharmacists, and pointed out the existing problems and deficiencies: unclear definition between disciplines, insufficient organic integration of knowledge, lack of practicability in the content selection of professional basic disciplines and so on. At the same time, corresponding revision suggestions are put forward for the revision ideas and contents of the syllabus of pharmaceutical examination: increase the degree of professional integration between disciplines and the breadth of relevant professional knowledge, start with clinical medication, emphasize people-centered pharmacy services, and pay attention to comprehensiveness and practicability. It is hoped to provide references for further improving the outline of the national licensed pharmacist qualification examination.

ABSTRACT

Objective:To analyze the functional connectivity (FC) characteristics of sensory motor network (SMN) in patients with bipolar disorder type Ⅰ (BD-Ⅰ) by independent component analysis (ICA), and explore the correlation between abnormal SMN and clinical symptoms.Methods:Eighteen patients with BD-Ⅰ (BD-Ⅰ group) and 20 matched normal controls (HC group) were included.Both groups received resting state fMRI (rs-fMRI) scanning.Based on ICA-fMRI data, one-sample t-test and two-sample t-test were used to analyze the components of SMN and to explore abnormal brain regions between the two groups.Functional network analysis (FNC) was also used to explore the functional connectivity between SMN and other brain networks.Pearson correlation analysis were conducted by SPSS 17.0 to measure the potential associations between intra-and inter-network functional connectivity and age, education, score of Bech-Rafaelsen mania rating scale (BRMS), score of positive and negative syndrome scale (PANSS) and other indicators. Results:In BD-Ⅰ group, the functional connection in the right paracentral lobule (MIN: x=8, y=-32, z=68, t=4.86, P<0.001) and the right postcentral gyrus (MIN: x=41, y=-26, z=53, t=3.33, P<0.001) in SMN were higher than those in HC group.Compared with HC group, the connectivity value in patients with BD-Ⅰ increased between SMN-DAN (0.247±0.073, -0.078±0.080, t=-2.974, P<0.01, FDR adjusted), while the connectivity value decreased between SMN-DMN(-0.037±0.054, 0.272±0.067, t=3.520, P<0.01, FDR adjusted) and between SMN-rFPN(-0.034±0.055, 0.231±0.070, t=2.939, P<0.01, FDR adjusted). Conclusion:The sensorimotor network of patients with BD-Ⅰ has abnormal functional connections within and between networks, and FC values in some networks are positively correlated with manic symptoms, which may be part of the neural mechanisms of patients with BD-Ⅰ.

ABSTRACT

Objective@#To provide a basis for college students mental health education and guidance, the impact of psychological aggression and corporal punishment college students experienced before the age of 10 on anxiety and health risk behaviors was explored.@*Methods@#In April and May 2019, the convenience sampling method was used to conduct a questionnaire survey among all the freshman and sophomore students of some majors in two universities in Harbin, including a total of 2 150 students. The contents of the questionnaire included general information survey, Parent-Child Conflict Tactics Scale (CTSPC),Self-Rating Anxiety Scale (SAS) and Adolescent Health related Risky Behavior Inventory (AHRBI).@*Results@#The percentage of college students who suffered psychological aggression and corporal punishment before the age of 10 was 80.05% and 58.37%. The detection rate of corporal punishment in boys and non-only child was significantly higher than that in girls and only child (P<0.05). The students who experienced psychological aggression and corporal punishment before the age of 10 had higher detection rates of anxiety state and six subscales of health risk behaviors than those who didn t experienced those (P<0.05). The Logistic regression analysis showed that, psychological aggression, place of residence, corporal punishment, and only child were positively associated with college students anxiety state (OR=1.85,1.53,1.49,1.34). The Logistic regression analysis showed that, psychological aggression, place of residence were positively associated with college students health-compromising behaviors (OR=1.60,1.26). Psychological aggression, gender factors, corporal punishment, and only child all influenced college students aggression and violence behavior (OR=1.75,1.66,1.63,1.28). Corporal punishment influenced college students suicide and self-injury behavior (OR=3.44). Gender factors and corporal punishment all influenced college students smoking and drinking behavior (OR=2.67,2.25).Corporal punishment and psychological aggression were positively associated with college students rule breaking behavior (OR=1.88,1.55). Corporal punishment, gender factors, and family income all influenced college students unprotected sexual behavior (OR=4.51,1.91,0.62).@*Conclusion@#Parental psychological aggression and corporal punishment college students experienced during childhood have a negative impact on their anxiety and health risk behaviors.

ABSTRACT

Objective:To investigate the safety and consistency of domestic inhaled allergen extracts in the diagnosis of allergic diseases in children.Methods:Nine thousand five hundred and sixty-three children diagnosed with allergic diseases from September 2018 to June 2020 in Children′s Hospital Affiliated to Capital Institute of Pedia-trics were selected in this study, and all of them were subjected to skin prick test(SPT), and 415 of them were subjected to serum specific IgE (sIgE) test at the same time.The adverse events during SPT were recorded and the consistency of the results between SPT and sIgE test was analyzed.Results:There were 14 cases with adverse events in 9 563 patients, and the overall incidence was 0.15%.The incidence of adverse events was 0.07% (2/2 581 cases) in the 1-5 years old group, 0.19% (12/6 197 cases) in the 6-11 years old and 0 in the 12-17 years old group.The severity of all these events was grade Ⅰ.Out of the 14 cases with adverse events, only 1 case was considered to be related to allergen preparations, with the incidence being 0.01% (1/9 563 cases). The Kappa index showed that the results of dermatophagoides pteronyssinus, dermatophagoides farinae, alternaria and artemisia measured by SPT and sIgE were almost the same.There was high consistency between tree pollens and ragweed ( P<0.01), and moderate consistency between aspergillus fumigatus ( P<0.01). When the results of sIgE were used as the diagnostic criteria, the Youden index for the results of SPT ranged from 0.76 to 0.89, with aspergillus fumigatus (0.76) and tree pollens mixture (0.79) as the lowest.The positive likelihood ratio for most of the inhaled allergens was more than 10 except for tree pollens mixture (7.12) and dermatophagoides farinae (9.10). The negative likelihood ratio for most of the inhaled allergens was less than or equal to 0.1 except for aspergillus fumigatus (0.19). Conclusions:The domestic inhaled allergen extracts had high safety in the clinical application of SPT, and their results of SPT had good consistency with those of serum sIgE, which was conducive to the diagnosis and evaluation of allergic diseases in children.

ABSTRACT OBJECTIVE:To establish the infusion sequence regulation procedure of PIVAS,and implement individual regulation. METHODS:By combining factors such as liquid volume of each batch,incompatibility between groups and specific solvents of specific drugs,intelligent prompt system and the artificial intelligent regulation system of infusion sequence were successively established to sequence the therapeutic drugs and adjuvant drugs used for the patients with different diagnoses and different purposes in different inpatient areas in a day,so as to realize intelligent control of individual infusion sequence. From Jan. to Sept. in 2017 and from Jan. to Sept. in 2019 as research stages before and after the intelligent control of individualized infusion sequence,the control effect was evaluated with the proportion of reasonable number of infusion priority cases,the proportion of appropriate number of elderly patients’batch liquid volume cases,the proportion of incompatibility cases between separate groups, the number and cost of flushing tubes as index. RESULTS:After regulating infusion sequence individually,the proportion of reasonable infusion priority rose from 5.56% to 98.72% ;the proportion of batch liquid quantity appropriate cases in elderly patients rose from 9.58% to 98.10%;the proportion which separated the incompatibility between groups rose from 41.03% to 99.12%;the number of washing tube dropped to(0.95±0.43)times/ward/d from(12.95±0.57)times/ward/d;the cost of washing tube could be saved 85 800 yuan/year. CONCLUSIONS:The implementation of individualized infusion sequence regulation promote safe and effective infusion,and provide reference for pharmacists of PIVAS in China to carry out precise pharmaceutical care.

ABSTRACT

OBJECTIVE:To evaluate th e eff ects of infusion sequence regulation on the promotion of intravenous rational drug use. METHODS :The intelligent control of infusion sequence was realized with Markov chain machine learning method on the basis of intelligent reminder manual labeling infusion sequence. Selected from Binhu Hospital of Hefei during Jan. to Sept. 2017, 164 800 inpatients’medication orders were collected as data before intelligent control of infusion sequence ,and 264 600 inpatients’ medication orders were collected as data after intelligent control of infusion sequence. The correct rate of incompatibility ,storage time limit ,administration frequency ,chronopharmacology,preventive drugs ,drug interaction ,auxiliary drugs and infusion sequence for intravenous irritant drugs were compared before and after regulation ;the times of flushing tube ,the cost of flushing tube infusion and the change of liquid property in the infusion tube were compared before and after regulation. RESULTS :The correct rate of incompatibility ,storage time limit ,administration frequency ,chronopharmacology,drug interaction and infusion sequence for auxiliary drugs after regulation were significantly higher than before regulation ;the correct rate of infusion sequence for preventive drugs and intravenous irritant drugs ,the times of flushing tube ,the cost of flushing tube infusion and the case number of the liquid change (proportion) in infusion tube were significantly lower than before regulation (P<0.001). CONCLUSIONS:Intelligent regulation of infusion sequence can improve the rationality of intravenous medication sequence to a certain extend.

Objective To compare the difference of cognitive fusion,empirical avoidance between patients with obsessive-compulsive disorder and healthy control group,and to explore the relationship between obsessive-compulsive score and cognitive fusion,empirical avoidance.Methods The cognitive fusion questionnaire(CFQ),acceptance and action questionnaire-2nd edition (AAQ-Ⅱ) and Yale-Brown scale for obsessive-compulsive symptoms (Y-BOCS) were used to investigate 100 subjects with obsessive-compulsive disorder (OCD) and 166 healthy controls.And the differences in cognitive fusion and empirical avoidance scores were compared between the two groups.The relationship of obsessive-compulsive symptoms and cognitive fusion,empirical avoidance was analyzed using multiple linear regression analysis.Results The scores of cognitive fusion ((49.89± 10.62) vs (33.88 ± 11.44),t =-11.345,P< 0.01) and empirical avoidance ((29.75±9.53) vs (21.59±7.03),t=-7.995,P<0.01) in obsessive-compulsive disorder group were significantly higher than those in healthy control group.There were significant differences in cognitive fusion and empirical avoidance in age variables (F=8.63,P<0.01).In AAQ-Ⅱ,item 2 (r=0.246,P< 0.05),item 6 (r=0.223,P<0.05) and total score (r=0.240,P<0.05) were positively correlated with the total score of Y-BOCS.Item 2 (r=0.311,P<0.01),item 3 (r=0.286,P<0.05),item 6 (r=0.248,P<0.05) and total score (r=0.229,P<0.05) were positively correlated with the scores of obsessive-thinking.In CFQ,item 2 (r=0.231,P<0.0) 5),item 4 (r=0.242,P< 0.05),item 7 (r =0.308,P< 0.05),item 8 (r =0.277,P<0.05) and item 9 (r=0.249,P<0.05) were positively correlated with the total score of Y-BOCS.Item 8 (r=0.261,P<0.05) was positively correlated with the scores of obsessive-thinking,item 7 (r=0.237,P<0.05) and item 9 (r=0.238,P<0.05) were positively correlated with scores of obsessive-compulsive behavior.When predicting total obsessive-compulsive scores,only CF item 7 of Q (B=1.827,P<0.01),item 3 (B=0.956,P<0.05),and item 6 of AAQ-Ⅱ (B=0.584,P<0.05) entered the equation with a joint explanatory variation of 19%.When predicting the score of obsessive-thinking,only item 2 of AAQ-Ⅱ (B=0.446,P<0.01) entered the equation,explaining the variance was 9.7%.In the prediction of obsessive-compulsive behavior,only item 9 (B=0.815,P<0.05) of CFQ entered the equation,and the explanatory variation was 5.6%.Conclusion Cognitive fusion in the patients with obsessive-compulsive disorder and the high level of empirical avoidance may be an important factor for the maintenance of the symptoms.

In AAQ-Ⅱ, item 2 (r=0.246, P<0.05), item 6 (r=0.223, P<0.05) and total score (r=0.240, P<0.05) were positively correlated with the total score of Y-BOCS.Item 2 (r=0.311, P<0.01), item 3 (r=0.286, P<0.05), item 6 (r=0.248, P<0.05) and total score (r=0.229, P<0.05) were positively correlated with the scores of obsessive-thinking.In CFQ, item 2 (r=0.231, P<0.0) 5), item 4 (r=0.242, P<0.05), item 7 (r=0.308, P<0.05), item 8 (r=0.277, P<0.05) and item 9 (r=0.249, P<0.05) were positively correlated with the total score of Y-BOCS.Item 8 (r=0.261, P<0.05) was positively correlated with the scores of obsessive-thinking, item 7 (r=0.237, P<0.05) and item 9 (r=0.238, P<0.05) were positively correlated with scores of obsessive-compulsive behavior.When predicting total obsessive-compulsive scores, only CF item 7 of Q (B=1.827, P<0.01), item 3 (B=0.956, P<0.05), and item 6 of AAQ-Ⅱ (B=0.584, P<0.05) entered the equation with a joint explanatory variation of 19%.When predicting the score of obsessive-thinking, only item 2 of AAQ-Ⅱ (B=0.446, P<0.01) entered the equation, explaining the variance was 9.7%.In the prediction of obsessive-compulsive behavior, only item 9 (B=0.815, P<0.05) of CFQ entered the equation, and the explanatory variation was 5.6%.@*Conclusion@#Cognitive fusion in the patients with obsessive-compulsive disorder and the high level of empirical avoidance may be an important factor for the maintenance of the symptoms.

ABSTRACT

Objective To investigate the correlation between plasma 25-hydroxyvitamin D(25-OHVD)levels and the CD4+/CD8+ ratio in elderly men in different states of glucose metabolism.Methods Clinical data of 206 elderly male patients at the geriatrics department of our hospital from April 2011 to August 2017 were collected and retrospectively analyzed.There were 85 patients with type 2 diabetes mellitus(T2DM),39 patients with impaired glucose tolerance(IGT) and 82 patients with normal glucose tolerance(NGT).Locally weighted regression and multivariate generalized linear regression models were used to analyze the correlation between plasma 25-OHVD levels and the CD4+/CD8+ ratio.Results There were significant differences in two-hour postprandial blood glucose and homeostasis model assessment of insulin resistance(HOMA-IR) between the NGT,IGT and T2DM groups (6.5 mmol/L,9.2 mmol/L vs.11.0 mmol/L,11.92 ± 10.57,16.46 ± 10.89 vs.32.67±7.39,respectively,P <0.01).Fasting plasma glucose and 2-hour postprandial insulin levels were higher in the IGT and T2DM groups than in the NGT group(5.1 mmol/L,6.3 mmol/L vs.4.9 mmol/L,440.5 nmol/L,367.8 nmol/L vs.255.4 nmol/L,P<0.05).The haemoglobin A1c(HbA1c)level was higher in the T2DM group than in the NGT and IGT groups(7.1% vs.5.5% and 5.8%,P<0.05).The 25-OHVD and CD4+ levels in the T2DM group were lower than those in the NGT and IGTgroups[11.6 μg/L vs.18.3 μg/L and 17.4 μg/L,(34.0±11.8)% vs.(40.7±10.5)% and (40.7±10.2)%,P<0.01],but were not significantly different between the NGT and IGT groups(P >0.05).The CD4+/CD8+ ratios in the T2DM and IGT groups were not significantly different from each other(1.4 vs.1.6,P>0.05),but were lower than that in the NGT group(2.7,P<0.01).After adjusting for the influence of related confounders,CD4+/CD8+ increased by 0.4 on average with each standard deviation(7.62 μg/L)increase of 25-OHVD.Compared with 25-OHVD patients in the first quartile,CD4+/CD8+ in the second,third and fourth quartile increased by 0.7,0.9 and 1.1 respectively on average,showing a significant positive correlation and a significant linear trend in the level of 25-OHVD and the CD4+/CD8+ ratio(P<0.01).Conclusions With the aggravation of glucose tolerance,plasma 25-OHVD levels and the CD4+/CD8+ ratio decrease by varying degrees in elderly men.The deficiency of plasma 25-OHVD may be an independent risk factor for a low CD4+ /CD8+ ratio.

ABSTRACT

Objective The use of sentinel lymph node biopsy (SLNB) following neoadjuvant chemotherapy (NAC) in patients presenting with clinically positive lymph nodes remains controversial.This study is aimed to invest if it is feasible to perform SLNB after NAC in case that axillary lymph nodes turned to clinical negative.Methods A computer-aided search of the literature regarding SLNB in clinically nodepositive breast cancer treated with NAC was carried out to identify the false negative rate (FNR),sentinel lymph node identification rate (IR).Results 16 articles were used in the analysis yielding 1 579 patients.The pooled estimate of FNR,IR,sensitivity and overall accurancy was 13.6%,91%,85.7%,and 92.8%,respectively.Conclusions SLNB after NAC in lymph node-positive patients,whose axillary lymph node was clinical negative after NAC,results in reasonably IR,but FNR was still high to be accepted.More refined patient selection and optimal techniques are expected to lower the FNR in this patient population.

ABSTRACT

OBJECTIVE:To refine the quality standard of pharmacy intravenous admixture service(PIVAS),and provide refer-ence for improving the work quality of PIVAS. METHODS:Through establishing quality management organization and developing quality standard rules,quality control system for PIVAS in our hospital was constructed and total quality management was conduct-ed. Numbers of quality problems before(Jul. 2013-Jun. 2014)and after(Jul. 2014-Jun. 2015)its implementation were compared, and the effects were evaluated. RESULTS:117 management systems and 14 link quality standards and rules were made,including staff behavior standards,quality standards for drug management,supervision and inspection of quality standards,etc. Numbers of quality problems dropped from 358 to 177 after the implementation,the ratio of dispensing errors accounted for the total dispensing declined from 0.35? to 0.17?(P<0.05). CONCLUSIONS:The construction of quality control system and the implementation of quality control standards and rules in PIVAS of our hospital has improved the quality of PIVAS work.

OBJECTIVE:To provide reference for standardizing dispensing and use of intravenous drug. METHODS:266 piec-es of intravenous drug instructions were collected in PIVAS of our hospital,and then the labeling information were statistically ana-lyzed,including dispensing method,diluted or dissolved solvent before preparation and storage time after preparation. RESULTS:Among 266 intravenous drug instructions,there were 45 kinds of finished product infusion varieties and 221 kinds of drugs needing to be prepared. Among these,there were only 30 pieces of drug instructions with labeling dispensing method,accounting for 13.57%;195 pieces of drug instructions stated solvent for dilution and 22 stated solvent for dissolution before preparation,account-ing for 88.24% and 19.13%;69 pieces of drug instructions stated storage time,accounting for 31.22%. CONCLUSIONS:The la-bel information of dispensing method and storage time in intravenous drug instructions is markedly insufficient or not clear. Drug ad-ministration departments are suggested to strengthen the instruction standardization management. Meanwhile,manufacturers should specify the relevant information in intravenous drug instructions.

ABSTRACT

Objective To evaluate effects of acitretin and interferon?α(INF?α)alone or in combination on the proliferative activity of and interleukin?15 expression in human cutaneous T?cell lymphoma Hut78 cells. Methods Cultured Hut78 cells were divided into several groups, including blank control group, negative control group, dimethyl sulphoxide (DMSO) group and experimental groups. Cells in experimental groups were additionally classified into several subgroups to be treated with acitretin(0.1-10μmol/L, acitretin groups)or INF?α(5 000-20 000 IU/ml, INF?αgroups) alone, or the combination of 1.0 μmol/L acitretin and IFN?α at concentrations of 5 000- 20 000 IU/ml (combination groups), for 24, 48 and 72 hours. Subsequently, cell counting kit 8(CCK8)assay was performed to assess the proliferative activity of Hut78 cells, and enzyme?linked immunosorbent assay(ELISA)to measure the expression of IL?15 in these cells. Results The proliferative activity of and IL?15 expression in Hut78 cells were both obviously suppressed in the acitretin groups and combination groups compared with the DMSO group, as well as in the INF?αgroups compared with the negative control group, and the inhibitory effects gradually increased with the increase in acitretin or INF?αconcentrations and treatment durations. As repeated measures analysis of variance revealed, there was a significant difference in both proliferation inhibition rates and IL?15 expression among different treatment durations and among different concentrations of acitretin or INF?α(all P<0.05), and there was an interaction effect between treatment durations and drug concentrations(all P<0.05). A significant difference was observed in both proliferation inhibition rates and IL?15 expression at 24, 48 and 72 hours when the 1.0?μmol/L acitretin+ 10 000/20 000?IU/ml IFN?αgroup was compared with the 1.0?μmol/L acitretin group and 10 000/20 000 IU/ml IFN?αgroup(all P<0.05). There was also a significant difference in IL?15 expression at 24, 48 and 72 hours between the 1.0?μmol/L acitretin+50 000?IU/ml IFN?αgroup and 5 000?IU/ml IFN?αgroup(all P<0.05). Conclusions Acitretin and IFN?αboth can inhibit the proliferation of and IL?15 expression in Hut78 cells, the inhibitory effects are enhanced with the increase in drug concentrations and treatment durations, and the combination of acitretin and IFN?α appears to have stronger inhibitory effects than acitretin or IFN?αalone.

ABSTRACT

Mesenchymal stem cells possess the ability of self-renewal and multiple differentiation potential, thus exert immunomodulatory effect during tissue repair. Mesenchymal stem cells can stimulate angiogenesis and promote tissue repair through transdifferentiation and secreting a variety of growth factors and cytokines. This review outlines the advances in the mechanism of mesenchymal stem cells in promoting wound healing, including alleviation of inflammatory response, induction of angiogenesis, and promotion of migration of mesenchymal stem cells to the site of tissue injury.
